LIABILITY WAIVER AND RELEASE AGREEMENT
Level-Up Jiu-Jitsu, LLC
1. ACKNOWLEDGMENT OF RISK
I understand that participation in Brazilian Jiu-Jitsu, grappling, wrestling, and related
martial arts activities involves inherent risks, including but not limited to:
- Sprains, strains, and fractures
- Dislocations and joint injuries
- Cuts, bruises, and abrasions
- Concussions or serious head injury
- Paralysis or death (in rare cases)
I acknowledge that these risks may arise from my own actions, the actions of others, or the
condition of the training environment.
2. ASSUMPTION OF RISK
I voluntarily choose to participate in these activities and assume full responsibility for any
risk of injury or loss, whether caused by negligence or otherwise.
3. RELEASE OF LIABILITY
I hereby release, waive, and discharge:
Level-Up Jiu-Jitsu, LLC, its owner(s), instructors, members, guests, and affiliates from any
and all claims, demands, or causes of action arising out of or related to any injury, damage,
or loss I may suffer as a result of participation in training activities.
This includes claims arising from negligence, to the fullest extent permitted by Michigan
law.
4. INDEMNIFICATION
I agree to indemnify and hold harmless Level-Up Jiu-Jitsu], LLC from any claims brought by
third parties as a result of my actions during participation.
5. MEDICAL FITNESS
I certify that:
- I am physically capable of participating in martial arts training
- I have no medical condition that would prevent safe participation, or I have disclosed such
conditions
6. MEDICAL TREATMENT CONSENT
In the event of injury, I authorize Level-Up Jiu-jitsu, LLC to obtain emergency medical
treatment on my behalf if necessary.
I understand that I am responsible for any medical costs incurred.
7. RULES AND CONDUCT
I agree to:
- Follow all instructions given by instructors
- Train in a safe and controlled manner
- Tap early and respect training partners
- Avoid reckless or dangerous behavior
8. EQUIPMENT AND FACILITY USE
I understand that training may take place in non-commercial or shared spaces and accept
any associated risks.
9. PHOTOGRAPHY AND MEDIA RELEASE (OPTIONAL)
I grant permission for Level-Up Jiu-Jitsu, LLC to use photographs or videos of me for
promotional purposes.
10. MINORS
If the participant is under 18, a parent or legal guardian must sign below.
11. GOVERNING LAW
This agreement shall be governed by the laws of the State of Michigan.
12. SEVERABILITY
If any portion of this agreement is found unenforceable, the remaining provisions remain in
effect.
13. ACKNOWLEDGMENT
I have read this agreement, fully understand its terms, and sign it voluntarily.
